If you would like to attend a Travel Clinic or speak to a Pharmacist regarding vaccinations and pending travel arrangements, you will need to complete one (or more) forms online and make a consultation payment before attending the clinic. Please click the button below to fill in the ‘Travel Clinic Risk Assessment Form‘. If you require require malaria tablets or a private flu vaccination, please click the button to complete the relevant pre-consultation form.
Upon completing the ‘Travel Form’ you will be required to pay an online £20 consultation fee for each person. Upon receipt of the form and payment, our pharmacists will contact you to arrange a time to visit the clinic. Any client who purchases vaccinations and/or antimalarials will only have to pay for their purchases. The consultation charge will be waivered at the end of the session for purchasing clients.
Clients are reminded to arrive at least 15 minutes before their appointment with a full written history of vaccinations (anyone under 18 must also bring their red book childhood immunisation history), allergies, medical conditions and medication they currently use (this must also be completed on the travel forms). We require the dates when you had vaccinations, all information about past and present medical conditions and medication (full name and dose) and all your allergies completed on the online form. This information should be obtained from your GP or Clinic prior to your appointment. If this information is not available at the time of your appointment, or if you arrive late for your appointment your appointment will be cancelled, as it would be clinically unsafe to continue with the clinic.
[We cannot vaccinate Pregnant, Immunocompromised, Sickle Cell Anaemia, Thalassaemia or HIV patients]
